Referring Provider Information
The primary pediatric health care provider (PCP) follows up with the family to discuss the evaluation results and recommendations, and continues to partner with and to assist the family in understanding the condition and its management, including further diagnostic evaluations (if applicable). Copies of assessment reports and diagnostic evaluations are provided to the family and to the PCP, and the family should be encouraged to bring these documents to all follow up visits. PCP offices should remind the family to do so when follow up visits are scheduled.
Primary Care clinicians may participate with schools and other agencies in developing service plans.
