Follow up visit using Medical Home Philosophy
The primary pediatric health care provider (PCP) follows up with the family to discuss the evaluation results and recommendations, and continues to partner with and to assist the family in understanding the condition and its management, including further diagnostic evaluations (if applicable). Copies of assessment reports and diagnostic evaluations are provided to the family and to the PCP, and the family should be encouraged to bring these documents to all follow up visits. PCP offices should remind the family to do so when follow up visits are scheduled.
The PCP should continue to monitor for health, developmental and/or behavioral concerns. For example, even if a developmental assessment turns out to show no significant delays, the fact that there had been a concern in the first place warrants follow-up developmental surveillance and screening within the next 1-3 months.
