• Medication Management Plan  

Medication management plan should include the following components:

• child's name, date of birth, and current date;
• diagnosis;
• treatment goals;
• name of medication;
• purpose of medication (as it relates to the treatment goals, not just the diagnosis);
• dosing and administration guidelines;
• significant side effects to watch for and what to do about them (i.e., when to stop medication; when to call physician; when to seek emergency medical assistance, etc.)
• other treatment plans (e.g., individual/family therapy, behavior goals and plans) follow-up plans (for primary physician, specialists, etc)
